    COVID led the industry to experiment with innovations that can reshape how care is delivered. The members of this year’s class of Top 25 Innovators have introduced ideas that engage consumers in new ways, lower healthcare costs and take unique approaches to managing population health.     Every year, Leadership Louisville Center seeks to address the most pertinent issue facing our city through the Bingham Fellows program. A reflection of the events of 2020, the selected topic chosen for the 2021 class was Belonging: Building a Culture of Inclusivity. The most diverse class in Bingham Fellows history came together to better understand what a more inclusive Louisville could look like, and arrived at one specific goal: to improve the lives of black residents in our city.

    Each of the projects that resulted from that goal was designed thoughtfully and strategically, and seek to increase access to:
    • Political empowerment through a Black agenda (The 3003 Project)
    • Opportunities to benefit from increasing supplier diversity (BEBA)
    • Investment capital for business startup and expansion (Bridge)

    All these build wealth and political power for Louisville’s Black residents. While none of them reverse the injustices of the past, they strive to provide equitable outcomes for the future.

The Gravity Project was initiated in November 2018 by SIREN with funding from the Robert Wood Johnson Foundation to convene broad stakeholder groups in identifying and harmonizing social risk factor data for interoperable electronic health information exchange. The project is a direct response to recommendations and calls to action around creating national standards for representing SDH data in EHRs. Growing evidence demonstrates strong links between social risk and an individual's health and health care utilization. This correlation has increasingly led health systems to incorporate social risk data into clinical decision making to improve health outcomes and help reduce costs. The Gravity Project seeks to identify coded data elements and associated value sets to represent social determinants of health data documented in EHRs across four clinical activities: screening, diagnosis, planning, and interventions. The project is focused on three specific social risk domains: food insecurity, housing instability and quality, and transportation access. These domains were selected based both on existing research linking these factors with health and utilization and on active experiments related to these domains, including in federal, state, or local demonstration projects.
