WCSU: Reporting Covid-19 Illness

If you have tested positive for Covid-19 please fill out and submit the form below.

 

After your positive test, you must isolate for 5 days. If you are a residential student, please return home to isolate off campus. All students who test positive and are isolating should email their professors to make arrangements for missed classes.


Please fill out the form in its entirety and remember to submit a copy of your positive test result.


If you are an employee, following your submission of this reporting form, you will be contacted by Fred Cratty in the Human Resources department with further instructions.


If you have not tested positive for COVID but have been exposed to someone diagnosed with COVID, please wear a mask when around others for a period of 10 days. The current CDC guideline recommends self-testing on day 6 post-exposure, or sooner if symptoms develop.


















If you have you been in close contact (i.e. closer than 6 feet for a total of 15 minutes or more over a 24 hours period) with anyone on campus within the 48 hour period prior to testing positive, please make sure that you inform these individuals of your positive test result so they can take the necessary precautions. If they have any questions, please refer them to the University’s COVID-19 Information website.
