JOB SUMMARY

The COVID Care Resource Coordinator position is responsible for conducting virtual needs check and connect at-risk COVID-19 positive patients and people in quarantine to community resources. This position is an on-call position. Must be available for shift work. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Identify the needs of assigned COVID-19 patients to receive care coordination.
2. Track all intake data and activities on the Salesforce system provided by LDH.
3. Search for local solutions to meet needs and resolve unmet needs by leveraging the Louisiana 211 database, LA 211 FAQ for COVID-19, and independent searches for customized solutions.
• This could include food banks, mental health services, visiting nurses, and other community programs helping patients in need and/or people in isolation or quarantine.
4. Escalate cases with the unresolved unmet need to supervisor in an effort to remedy.
5. Communicate new or changing information needed for the LA 211 database to 211helpdesk@launitedway.org.
6. Maintain daily contact with the supervisor.
7. Provides clients with personalized attention while working towards performance targets provided by Volunteers of America Southeast Louisiana.

KNOWLEDGE, SKILLS, ABILITIES:

This position requires knowledge, skills, and abilities equivalent to:

• Degreed or non-degreed
• Social work experience required
• Perform compliance and quality assurance audits on Supportive Service Programs
• Requires managing case as needed for compliance
• Familiarity with experience working in Louisiana health system, experience working with community resources and social services in different parts of Louisiana that can inform connecting patients with COVID and their families to local resources.
• Take steps to identify the appropriate action to address identified needs and coordinate efforts to ensure these needs can be met within __72__ hours (or days) depending on the level of need and available resources.
• Maintain communication with Contact Tracer Supervisors and Regional Medical Directors to ensure seamless transitions and receive updates on resources available or changes to workflow.
• Performs other duties as assigned.
• Bilingual preferred
• Ability to exhibit a professional, positive attitude, and work ethic.
• Excellent interpersonal skills required and the ability to interact professionally with culturally diverse individuals during a time of crisis and distress.
• Excellent organizational and communication skills.
• Critical thinking and sound judgment required
• Ability to handle confidential information with discretion and professionalism
• Proficiency with computers and
• Technology requirements: PC with Windows 10, Antivirus Protection: Windows Defender and Windows Firewall; or Mac with Apple OS X10.13, Antivirus Protection: Sophos and personal mobile device to use for this job. A headset is preferred.
