This form is required for members to register for the LASTO 2020 Spring Conference. Each attendee, including members, are required to complete and submit this form to ensure accurate records and conference provisions. Please complete this form for each person attending.

Members are required to either mail your registration fee to Ms. Debbie Buckley, (510 Guthrie Road, Sterlington, LA 71280), or pay using a credit card on PayPal at : http://lasto.org for Members Fee button or pay at the registration table. Please be aware that the fee is $25 higher if paying at the event registration table. Please indicate your payment method before clicking submit to finalize your register.
