Request Help

If you are seeking advocacy/legal assistance on behalf of yourself or someone else, please complete DRM's Online Intake Form. Our intake staff will review your request, in consultation with a managing attorney, to determine whether we may be able to assist. It is helpful if you can provide as much information about your situation as possible,  However, if we have additional questions, someone will contact you to set up a telephone intake appointment.  When completing this form, please be careful not to share sensitive information, such as your Social Security Number.

Please note that DRM is not able to accept all requests for assistance. Information about the types of issues we may be able to assist with is available on our What We Do page. This page also includes information and resources for those issues that we are unable to assist with.

If you submit an online intake, you do not need to call our office and schedule a telephone intake.  If we need more information, someone will contact you at the email/number provided.

The information you provide below is confidential. Submitting an Online Intake and requesting assistance does not establish an attorney/client relationship.

Please provide the date of birth for the person you are contacting us about. If you do not know their date of birth, you may leave this field blank.
If you are not contacting us for yourself, who is the person you are seeking assistance for?
Name of individual, employer, service provider, etc.
Please tell us about the issue that you are looking for help with, including how you would like to see this situation resolved.
